Doctor visits

Dring my adult life, I have had four GPs, had two ambulance rides, and met a number of A & E doctors. Without exception their first comment has always been: "Hello, and how are you?" My initial reaction is always to say: "Fine, thank you, and how are you?" Then realization hits that if this were true, I wouldn't be there. So I then have to hastily backtrack to explain why I am there. Do all doctors do this? Is it taught at medical school?
